1. Empathize
I conducted research through interviews with doctors, therapists, and medical assistants. Key findings included:
They feel overwhelmed by the manual note-taking process during or after sessions.
They need to ensure notes comply with SOAP (Subjective, Objective, Assessment, Plan) standards.
They want a system that is secure, compliant (HIPAA/ GDPR), and easy to use.
Scheduling and managing patient records is often done with multiple tools.
2. Define
I defined the core problem:
Medical professionals need a centralized, AI- driven platform that simplifies patient documentation, integrates care-specific features, and reduces administrative burden while maintaining compliance.
3. Ideate
I brainstormed and mapped out the must-have features:
Record: Doctors record live sessions (audio/video).
AI-Generated Notes: Automatic SOAP notes (Summary, Objective, Assessment, Plan) + patient instructions + transcript.
Dashboard: Overview of appointments, tasks, and recent notes.
Templates: Ready-to-use or custom templates for specific treatments (e.g., Psychiatry, Therapy, Addiction Medicine).
Patients: Secure records, history, demographics.
Schedule: Book, reschedule, and manage patient sessions.
4. Prototype
I designed a clean and professional web app with:
Intuitive navigation for quick access to records, templates, patients, and schedules.
Minimal but trustworthy color palette (blue and red shades for healthcare).
AI integration that highlights editable text for quick doctor review.
Mobile responsiveness for doctors who switch devices often.
5. Test
I tested the prototype with 5 medical practitioners.
Feedback:
Doctors loved the AI- generated SOAP format but wanted the ability to edit before saving.
They requested tagging and categorization for notes.
Scheduling: Needed reminder notifications for both doctors and patients.
I iterated the design based on feedback.
User Persona
User Research
To understand the needs of medical practitioners across psychiatry, therapy, addiction medicine, and palliative care, I conducted:
5 in-depth interviews with psychiatrists, therapists, and nurse practitioners.
2 contextual observing how practitioners currently handle documentation during/after sessions.
Competitive analysis of existing AI note-taking tools (e.g. Augmedix, DeepScribe, Nuance DAX).
Key Insights
Most practitioners struggle with documentation time taking away from patient interaction.
They often feel anxious about accuracy and compliance when using automated tools.
Many already rely on personal note templates, but find them hard to organize and reuse.
Trust in AI is low unless transcripts and edit control are provided.
Pain points
Based on the research, I identified four core pain points:
1. Documentation Burden: Practitioners spend 30-45 minutes after each session writing SOAP notes. This reduces time available for patient care and increases burnout.
2. Fragmented Workflows: Notes, schedules, and patient history are often spread across multiple tools (EHR, spreadsheets, paper). Switching contexts wastes time and increases errors.
3. Accuracy & Compliance Concerns: Fear that AI might misinterpret medical terms or omit critical details. Practitioners worry about legal and ethical risks if documentation isn’t precise.
4. Low Reusability of Templates: Many clinicians have personal note templates but lack a centralized system to store, reuse, and adapt them for different cases, leading to repetitive work.

Designing this AI-powered platform showed me how crucial it is to balance AI automation with human oversight in healthcare. Doctors wanted control over AI outputs but appreciated the huge time-saving benefits.
